How to Schedule an Appointment
To schedule a new patient appointment with us you have 3 options. You can either call our office number at 316-542-3400, schedule online, or send us a message on our Facebook page- Hague Chiropractic.
New Patient Forms
On your first visit, we will need you to fill out intake forms. Then the doctor will review your complaints and get you the treatments you need on the same day.

School Sports Physicals - We do have all the necessary paperwork here in the office.

Do you accept insurance?
Hague Chiropractic does not accept insurance but you are able to use your FSA and HSA accounts for our services.
